Some states require that notaries log their clients in a journal. Other states don’t require it, but it’s still a good idea. Whether your state requires it or not, getting in the habit of logging everything can help ensure that you don’t end up in any kind of trouble if you’re questioned about something later.
In general, if you perform any kind of service you should log it. If, for some reason, you deny service to a client you should log it, and you should clearly state what service was to be performed and why you refused it. If there is more than one document for a signer, you should log each different document and each different signer. If you don’t charge a person for some reason, you should state why.
Also, make sure that the person signing the document also signs your journal and puts his or her thumbprint there, if possible. This is great proof in court that the person who signed the document was actually present, and that can help if there is a dispute about the document that was signed.

A true and exact copy is exactly that – one that has not been changed or tampered with in any way. The reason for this wording is that a true copy could be a copy that was made of a document and then had things changed on it. If it’s a true and exact copy, it means that it is a copy of the original document and has not been altered in any way. If you got out the original document and looked at it it wouldn’t look any different, at all, from the copy.
When you stamp something with the true and exact copy stamp it means that you are personally certifying that the copy is identical in every way to the original and that it has not been tampered with or changed. That sounds like no big deal, but it could matter a lot, especially in the areas of dollar amounts, birthdates, and other things that are vital to the signing and enforcement of specific documents.
The true and exact stamp is very important, so be sure that the copy you stamp with it really meets that criteria.
